Inadequate breast milk production often becomes a major obstacle to the successful exclusive breastfeeding of postpartum mothers, especially those who undergo cesarean section (SC) surgery. Physical and psychological factors, including post-operative pain and stress, can inhibit the let-down reflex of breast milk. Breast massage using the Oketani Breast Massage (OBM) technique is believed to improve milk flow by stimulating the hormones oxytocin and prolactin. This study aimed to evaluate the effectiveness of applying Oketani Breast Massage in improving breast milk production smoothness in post-SC mothers. This study used a case study design with subjects being postpartum mothers on the second day after SC who experienced difficulties in milk production. OBM intervention was performed twice daily for three days, accompanied by education on milk expression techniques and oxytocin massage by the husband as support. Data were collected through observation of mother and baby conditions, measurement of breast milk production frequency and quality, and the mother's psychological responses. Results showed a significant increase in milk flow after OBM application. The mother's breasts became softer, milk began to flow during breastfeeding, the baby's urination frequency increased, and the mother felt more confident and experienced reduced anxiety related to breastfeeding. The conclusion is that Oketani Breast Massage is an effective intervention to help increase breast milk production in post-SC mothers, with educational support and active family involvement as key supporting factors.
